Job summary

AuthoraCare Collective seeks a Social Worker Home Care in the Burlington area to provide comprehensive psychosocial assessments and advanced counseling for hospice patients and families. You will work within an interdisciplinary team to plan end-of-life care and connect families with resources.

The role requires a Master’s in social work and licensure (LCSW/LCSWA) with at least one year in mental health or medical settings.

Qualifications

• Master's degree in social work
• LCSW OR LCSWA (or in process) required
• One year of social work experience in mental health or medical setting
• Valid state-issued driver’s license; must have own transportation and automobile liability insurance

Responsibilities

• Prepares psychosocial assessments.
• Develops and carries out individualized plans of care related to grief, loss, adjustment, care, and end of life planning needs of patients, families, and caregivers.
• Identifies appropriate resources to meet patient/caregiver/family needs.
• Serves as an advocate for patients and families.
• Actively participates with the interdisciplinary group (IDG).
• Maintains notes and patient records per licensure and agency standards.
