Job summary

Optimal Care is seeking a Hospice Medical Social Worker to deliver compassionate psychosocial care for patients and families facing end‑of‑life illness. You’ll conduct assessments, provide counseling, and support advance care planning within an interdisciplinary team to ensure dignity and emotional well‑being.

The role requires LMSW/LCSW licensure, a minimum of 1 year of relevant experience, and willingness to travel within the service area to patients’ homes, facilities, and community settings.

Qualifications

• MSW degree from CSWE‑accredited program and state licensure as LMSW or LCSW.
• Minimum 1 year of licensed MSW experience.
• Willingness to travel to homes, facilities, and community settings.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Provide comprehensive psychosocial assessments and crisis intervention.
• Deliver counseling and support advance care planning conversations.
• Collaborate with nurses, physicians and hospice team to integrate psychosocial needs into each plan of care.
• Support patients and families through end‑of‑life transitions and bereavement readiness.
