Job summary

Life Care Home Health Family is seeking a licensed Master of Social Work to oversee Medical Social Workers in hospice care. The role involves psychosocial assessments, care coordination with the Interdisciplinary Group, and support for patients and families.

The supervising MSW will ensure compliance with standards, participate in IDG meetings, and provide counseling on end-of-life issues, discharge planning, and bereavement support.

Qualifications

• MSW degree required with LMSW/LCSW licensure.
• Graduate degree in social work.
• Must meet Texas/Jurisprudence exam or state equivalent requirements.

Responsibilities

• Supervise Medical Social Workers as per standards.
• Assess patient/family psychosocial needs within 5 days of hospice election.
• Participate in IDG/Team meetings and care planning.
• Provide counseling on end-of-life, grief, and advance directives.
• Attend on-call rotations and ensure timely paperwork.
