Job summary

Phoenix Home Care & Hospice is seeking a Hospice Social Worker to provide in-home counseling and support to patients and families in Hutchinson, KS and surrounding areas. The role collaborates with the IDG and coordinates referrals while promoting comfort and quality of life at the end of life.

Qualified candidates hold an MSW or LCSW, with hospice experience preferred and strong communication and teamwork skills to connect patients with needed resources and services.

Qualifications

• Master’s in Social Work (MSW) or LCSW required.
• Hospice experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Professional, dependable, and organized

Responsibilities

• Provide counseling and support to patients and families in the home
• Assist with advance directives and community resources
• Participate in the hospice interdisciplinary team (IDG)
• Coordinate referrals and additional support services
• Maintain positive relationships with patients, families, staff, and referral partners
