Job summary

Qualifications
- Master's in social work or related field with applicable state licensure.
- 2+ years of experience in social work, hospice preferred.
- LSW license and CPR certification required.
Responsibilities
- Coordinate hospice services with facility/hospital case managers.
- Assess psychosocial needs and develop care plans for patients and families.
- Assist with obtaining financial resources and Advance Directives.
- Document in electronic medical record and communicate with care team.
