Job summary

Gulfside Healthcare Services in Florida is seeking a Hospice Social Worker to provide psychosocial support and education to patients and families in private residences, assisted living facilities and skilled nursing homes. You will perform assessments, develop care plans, and collaborate with an interdisciplinary team.

The role requires an MSW and offers flexibility with hours, including evenings/weekends as needed.

Qualifications

• MSW from an accredited school/university
• One year post-MSW experience in hospital, home health, or hospice preferred
• Must possess strong ethical standards and professional demeanor
• Experience in individual, family, and group treatment modalities
• Knowledge of human development, end-of-life care, and family systems
• Able to remain calm and troubleshoot patient/family or staff crises
• Must have reliable transportation and provide proof of automobile insurance and a valid driver's license

Responsibilities

• Practices Social Work in a manner that models professional and agency ethics, values and integrity and complies with the letter and spirit of legal aspects
• Adheres to confidentiality regarding patients, families, and staff
• Complies with all applicable laws and regulations with respect to collaborative agreements
• Is empathic and compassionate with a patient-centered attitude towards accepting death as a part of life and enhancing quality of life for patients
• Conducts initial and ongoing psychosocial, spiritual, and pre-bereavement assessment of patient/families
• Assists with development and implementation of patient/family personalized care plans including counseling and referrals to GHS and other resources
• Provides case management services relating to psychosocial needs for patients/families
• Identifies and works with high risk and suicidal patients/families and other non-medical crises
• Completes clinical documentation, psychosocial assessments, clinical notes, and bereavement assessments
• Attends interdisciplinary team meetings and participates in team conferences
• Ability to work flexible hours including evenings and/or weekends as needed
• Performs other duties as assigned by Social Work Manager/Director
• Demonstrates professional growth through training and development
• Adheres to policies and represents GHS in the community
• Promotes the company through community partnerships and professional organizations
