Job summary

Haven Hospice in Florida seeks a compassionate Social Worker to join our team serving Palatka, FL and surrounding areas. You will assess psychosocial needs, provide counseling, educate families, and coordinate with community resources to support end-of-life care.

The role requires a CSWE-accredited MSW or BS in Social Work, at least one year in healthcare (hospice preferred). A valid driver’s license with insurance is required.

Qualifications

• Master’s degree in social work (MSW) accredited by the Council on Social Work Education OR a bachelor’s degree in social work (BSW) accredited by the CSWE.
• Minimum of one year of health care experience; hospice experience preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure to practice in the state of operation; must be a licensed driver with an insured vehicle.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans when needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support funeral, financial, and legal planning with families.
