Job summary

Haven Hospice in DeLand, FL is seeking a compassionate Social Worker to provide emotional support, counseling, and resource coordination for patients facing terminal illness.

You will assess psychosocial needs, educate families, and collaborate with the care team to deliver holistic, dignified end-of-life care in the home setting. A CSWE-accredited MSW/BSW and state licensure are required.

Qualifications

• MSW or BSW accredited by CSWE.
• Minimum of one year healthcare experience; hospice experience preferred.
• Current licensure to practice in the state.
• Must have a valid driver's license with a vehicle insured as required.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans as needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support funeral, financial, and legal planning with families.

Haven Hospice is seeking a passionate, dedicated Social Worker to join our team inDeland, FL. Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect.

Qualifications

• A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).
• Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
