Counseling and EMR documentation are essential parts of the role. Required: Master's in social work, 2+ years of social work experience, LSW, BLS, and a valid driver's license.

Qualifications

• Master's degree in social work or related field (required).
• 2+ years of experience in social work (required).
• Basic Life Support (BLS) certification (required).
• Licensed Social Worker (LSW) credential (required).
• Driver's license (required).

Responsibilities

• Coordinate hospice services with facility/hospital case managers.
• Assess psychosocial needs of patients and families; develop care plans.
• Conduct initial assessments at patient residences.
• Educate patients and families on Advance Directives.
• Provide individual or group counseling and document in EMR.
