Job summary

Catholic Home Health Services is seeking a Social Worker to provide direct and indirect social work services, primarily in patients' homes. The role involves visiting patients, assisting with psychosocial needs, and contributing to the care plan with family counseling and community referrals.

Must hold a Master's in Social Work and ideally an active Florida license; 1–2 years of healthcare social work experience preferred, with proficiency in clinical software and English language skills.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Active Florida social work license preferred.
• 1–2 years of social work experience in health care; home health preferred.
• Knowledge of AHCA regulations and Medicare COPs.
• Familiarity with public health administration or supervision preferred.
• Proficiency with clinical/office software.
• Must be able to read, write and understand English.

Responsibilities

• Instruct, treat, observe and evaluate patients with social/emotional issues affecting health.
• Contribute to physician Plan of Care development and re-evaluation.
• Counsel patients and families on coping with illness-related psychosocial stress.
• Conduct psychosocial assessments for patients and families.
• Assess needs for pre-bereavement/anticipatory grief.
• Implement Care Plan provisions to meet psychosocial goals (counseling, referrals, problem solving).
• Follow infection control policies and OSHA guidance in care activities.

Education

Master's Degree in Social Work

Florida state license (active preferred)

1-2 years health care social work experience

AHCA Regulations / Medicare COP knowledge

Public health administration / supervision knowledge (preferred)

Computer office/clinical software literacy

English language proficiency
