AccentCare is seeking a Social Worker / MSW for hospice in Largo, MD. This full-time role centers on providing compassionate social work inside a multi-disciplinary hospice team, with emphasis on discharge planning and family education.

Qualifications

• Master’s Degree from an accredited school of Social Work or related field.
• Valid license/registration/permit to practice in state(s) of agency operation if required.
• At least 1 year of professional social work experience in a healthcare setting (2+ years preferred).
• Medicare/Medicaid/Insurance knowledge preferred.

Responsibilities

• Assist the core members of the hospice team in understanding social, spiritual, and emotional factors to establish a plan of care.
• Participate in development and review of the plan of care for all patients.
• Assess social, spiritual, and emotional needs to support patients and caregivers.
• Educate on advance directives and advance care planning.
• Utilize community resources to support patients and families.
• Support the bereavement program.
• Provide discharge planning related to level-of-care changes.
