Job summary

AccentCare Inc. in Washington, DC is seeking a part-time Hospice Social Worker (MSW I) to support patients and families at MedStar Washington Hospital Center. You will work 8am-5pm on M/W/F, providing compassionate social work services within the hospice team.

This role requires a Master’s in Social Work and a state license, with preferred experience in hospice or healthcare. You will collaborate with an interdisciplinary team to plan care, address psychosocial needs, and assist with discharge

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Valid licensure/registration to practice in the state of operation.
• Experience or exposure to hospice or healthcare settings preferred.
• Medicare/Medicaid/Insurance familiarity preferred.

Responsibilities

• Assist the hospice team in developing plans of care that respect patient dignity and well-being.
• Participate in interdisciplinary team meetings and care plan reviews.
• Assess social, spiritual and emotional needs to support patients and caregivers.
• Educate families on advance directives and care planning.
• Support discharge planning and community placement coordination.
• Provide bereavement support as part of the program.
