Job summary

AccentCare in Chisago City, MN seeks a Social Worker / MSW for hospice to join a full-time team. You will collaborate with the interdisciplinary group to assess social, emotional, and spiritual needs, develop plans of care, and support families through the hospice journey.

Qualifications

• Master’s Degree from accredited school of Social Work; Valid license/registration to practice in state(s) of operation if required
• Medicare/Medicaid/Insurance specialty preferred
• 1 year of professional social work experience (post-graduate) in healthcare; 2+ years preferred
• Experience or education in grief counseling preferred
• Discharge planning experience preferred

Responsibilities

• Assist core hospice team in understanding social, spiritual, and emotional factors to establish a plan of care
• Participate as a member of the interdisciplinary team in development and review of the plan of care
• Assess social, spiritual, and emotional needs to estimate capacity to cope with illness and terminal diagnosis
• Prepare patient and family to cope with changes and advance care planning
• Utilize resources to support patient and family within illness limitations
• Support the bereavement program
• Provide discharge planning related to level-of-care changes or community placement/location-of-care
