Capital Caring Health is seeking a Hospice Social Worker to provide psychosocial support to patients and families in accordance with the care plan. Responsibilities include coordinating care, addressing emotional needs, and delivering counseling in end-of-life settings.

The role requires an MSW from an accredited program, at least 2 years of clinical social work experience, and licensure in DC, MD, or VA. Travel to patients’ homes and facilities is expected.

Competenze

• MSW from an accredited Social Work school is required.
• Minimum 2 years of MSW experience in an interdisciplinary or health care setting.
• Licensure from the governing body in DC, MD, or VA is required.

Mansioni

• Coordinate all aspects of the patient’s care in accordance with the plan of care.
• Identify and address emotional and biopsychosocial needs of patients with advanced illness at end of life.
• Provide counseling and psychosocial assessment; support patient, family, and caregivers.
