The position requires a CSWE-accredited Master’s Degree in Social Work, valid driver’s license, and a willingness to participate in CPR training if not already certified.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Licensed Social Worker in the state of residence (preferred).
• Current CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Assists the patient and family to understand social and emotional factors affecting health status.
• Contributes to a comprehensive Plan of Care for patients on a daily basis.
• Instructs health care team on community resources available to assist patients.
