Job summary

Qualifications

• Master's Degree in Social Work required.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• Preferred: 1+ year(s) of social work experience in health care; CPR certification; Licensed Social Worker in state of residence.

Responsibilities

• Assists patient, family, physician, and care team to understand social and emotional factors affecting health.
• Contributes to development of a comprehensive Plan of Care for patients daily.
• Instructs health care team on community resources to support patients.
