Job summary

Responsibilities include developing a Plan of Care, assisting with resource navigation, and educating team on community resources to support patients and families.

Qualifications

• Master's Degree from an accredited School of Social Work.
• Current Driver's License with access to dependable transportation.

Responsibilities

• Assist the patient, family, physician and health care team to understand significant social and emotional factors related to the patient's health status.
• Contribute to the development of a comprehensive, integrated Plan of Care for patients daily.
• Instruct health care team members on community resources available to assist patients as needed.
