Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Social Worker to join our Indiana, PA team. You will provide emotional support, counseling, and resource coordination to patients facing terminal illness, ensuring dignified, holistic care.

With a focus on home-based services, you will assess needs, educate families, and collaborate with a multidisciplinary team to support patients and their loved ones through grief and advance care planning.

Qualifications

• Master's degree in social work (MSW) accredited by CSWE or Bachelor's in social work (BSW) accredited by CSWE
• Minimum 1 year experience in health care; hospice experience preferred
• Current licensure per Medicare guidelines to practice in state of operation

Responsibilities

• Assess psychosocial needs of patients and families
• Evaluate home safety; assist with alternative care plans when needed
• Provide counseling, crisis intervention, and emotional support
• Maintain accurate and timely clinical documentation
• Educate and assist with advance directives
• Provide referrals and practical support for patients and families
• Serve as liaison with community resources and agencies
• Collaborate with team to support holistic patient care
• Support funeral, financial, and legal planning with families
