Qualifications

• MSW degree from CSWE-accredited program
• Current state license as a social worker
• 2+ years clinical work experience in healthcare or hospice
• Reliable transportation and ability to lift up to 25 lbs

Responsibilities

• Perform psychosocial evaluations for patients and families
• Provide ongoing counseling to patients and families
• Offer emotional support during end-of-life care
• Coordinate with the care planning team to align with the plan of care
• Support patients and families through the hospice journey
