Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• MS degree in social work from an accredited school of social work.
• Current state license as a social worker.
• 2+ years of clinical work experience, preferably in healthcare or hospice.

Responsibilities

• Conduct psychosocial evaluations and ongoing counseling for patients and families.
• Provide emotional support to patients and families during the end-of-life journey.
• Collaborate with the care team to implement the plan of care.
