Job summary

Agape Care Group in Oklahoma is seeking a dedicated hospice medical social worker to provide psychosocial evaluations and ongoing counseling to patients and families facing end-of-life decisions.

You will work with the plan of care to deliver emotional support, coordinate with the care team, and contribute to a compassionate, inclusive workplace that offers professional development and growth opportunities.

Qualifications

• MS degree in social work from an accredited school of social work approved by the Council of Social Work Education.
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice.
• Licensure: Current state license as a social worker.
• Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs.

Responsibilities

• Perform psychosocial evaluations for patients and families.
• Provide ongoing counseling and support during end-of-life care.
• Collaborate with the care team to implement the plan of care.
