Agape Care Group is seeking a compassionate hospice medical social worker to conduct psychosocial evaluations and provide ongoing counseling to patients and families on the end-of-life journey.

You will support care planning, collaborate with the leadership team, and help ensure a meaningful, comforting experience for those we serve, with opportunities for professional development and advancement.

Qualifications

• MSW degree from CSWE-accredited program.
• Current state social work licensure.
• 2+ years clinical experience, preferably in healthcare or hospice.
• Reliable transportation; ability to sit, stand, bend, lift at least 25 lbs.

Responsibilities

• Conduct psychosocial evaluations.
• Provide ongoing counseling to patients and families.
• Assist with care planning per the plan of care.
• Support families during end-of-life journey.
