ACG Hospice is seeking a dedicated Social Worker to support patients and families through advanced illness with compassionate psychosocial care. You will perform psychosocial evaluations, provide ongoing counseling, and collaborate with the care team to align the plan of care with patient goals.

Qualifications

• MSW from CSWE-accredited program
• Licensed social worker in the state
• At least 2 years of clinical experience, healthcare or hospice preferred
• Must have reliable transportation and ability to lift up to 25 lbs

Responsibilities

• Perform psychosocial evaluations for patients and families
• Provide ongoing counseling during end-of-life care
• Collaborate with the care team to align the plan of care with patient goals
• Support emotional well-being and family coping throughout illness trajectory
