Job summary

Agape Care Group seeks a compassionate hospice social worker to perform psychosocial evaluations and provide ongoing counseling for patients and families. You will support emotional well-being during the end-of-life journey and collaborate with the care team to follow the plan of care.

The role requires an MSW, a current state social work license, and 2+ years of clinical experience, preferably in healthcare or hospice, with reliable transportation and physical ability to lift 25 lbs.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.

Responsibilities

• Perform psychosocial evaluations and ongoing counseling for patients and families
• Provide emotional support to patients and families during end-of-life journey
• Work in accordance with the plan of care and coordinate with the care team.
