Job summary

Agape Care Group is seeking a dedicated hospice medical social worker to perform psychosocial evaluations and ongoing counseling for patients and families during end-of-life care. You will provide emotional support in alignment with the plan of care and collaborate with our leadership team on professional development and advancement opportunities.

Qualifications

• Heart to serve patients and families and a passion for providing the best possible care
• MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice
• Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.

Responsibilities

• Perform psychosocial evaluations and ongoing counseling of patients and families during the end-of-life journey.
• Provide emotional support to patients and families when it's needed most.
• Work in accordance with the plan of care to support patients and families.”
