Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families and coordinate supportive resources within an interdisciplinary team.

The role emphasizes hospice experience (1+ year), bereavement planning, and education to patients and families while maintaining timely documentation in the EMR. A Master’s in Social Work is required, with IL IDFPR licensure considerations.

Qualifications

• Master's in Social Work required.
• Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) in Illinois required.
• Driver's License and proof of auto insurance required.
• Hospice experience of 1+ years required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental factors.
• Develop a bereavement plan of care addressing family needs.
• Educate patients and families, respecting learning needs.
• Document assessments and interventions in the EMR.
• Collaborate with IDT to develop and implement social work care plans.
