Qualifications

• MSW from CSWE-accredited program.
• Current state social work licensure.
• Healthcare experience preferred.
• Ability to coordinate with IDT.

Responsibilities

• Explain hospice services and benefits to patients and families.
• Collaborate with IDT to develop care plans.
• Provide psychosocial support and counseling.
• Complete and file timely documentation.
• Assist in long-term care placement and advance planning.
