Job summary

Compassus in West Plains, MO seeks a dedicated Hospice Social Worker to work with the Interdisciplinary Team to address the psychosocial needs of patients and families, enhance quality of life, and support care transitions across settings.

The role emphasizes compassionate counseling, care coordination, and resource linkage, while upholding ethical standards and cultural sensitivity within hospice care. Experience in healthcare and MSW preferred.

Qualifications

• MSW degree from a CSWE-accredited program (or equivalent)
• Healthcare experience preferred
• Licensure as required by state Missouri for Social Workers

Responsibilities

• Explain hospice services and Medicare benefits to patients and families and obtain informed consent
• Participate as a member of the IDT and contribute to the plan of care
• Provide psycho-social support and counseling to patients and families
• Coordinate with external counseling resources as needed
• Complete initial and ongoing psycho-social assessments and address financial, housing, caregiver stressors
• Facilitate advance planning and end-of-life documentation
