Four Seasons Compassion for Life seeks a Social Worker to provide psychosocial services and support for hospice patients and families. You will manage a caseload and ensure care aligns with mission, policies, and procedures.

The role includes assessments, care planning, community referrals, counseling, and assisting with financial and bereavement needs. Must have a degree in social work and healthcare experience.

Formación

• Bachelor’s in Social Work (BSW) or Master’s in Social Work (MSW).
• 1–3 years experience within a healthcare setting.
• Valid driver’s license and proof of automobile insurance.

Responsabilidades

• Perform initial and ongoing psychosocial assessments of patients/family/caregiver.
• Develop and evaluate a plan of care for each patient/family/caregiver.
• Serve as liaison to community health, welfare and social agencies; make referrals.
