Job summary

Four Seasons Compassion for Life seeks a Social Worker to provide psychosocial services for hospice patients and families. You will assess needs, develop care plans, coordinate community referrals, and support discharge planning while collaborating with the IDG team.

Requirements include a BSW or MSW and 1–3 years in healthcare; NC licensure is preferred. Full-time/benefits role based in Buncombe County with a Monday–Friday schedule.

Qualifications

• Education: BSW or MSW required.
• 1-3 years experience in healthcare setting.
• Valid NC driver’s license and proof of automobile insurance.
• Valid NC Clinical Social Worker license preferred.

Responsibilities

• Perform psychosocial assessments of patients and families.
• Develop and evaluate care plans for assigned caseload.
• Coordinate referrals to community agencies and services.
• Provide counseling and education to patients and families.
• Assist with discharge planning and financial aspects of care.
