Job summary

Four Seasons Compassion for Life in Western North Carolina seeks a Social Worker to provide psychosocial services to hospice patients and families, managing a caseload with varying needs across home and facility settings. You will perform assessments, develop care plans, coordinate with community agencies, and support families through discharge planning, bereavement, and end-of-life decisions.

Qualifications

• Bachelor’s degree in Social Work (BSW) or Master’s in Social Work (MSW).
• 1–3 years experience in a healthcare setting.
• Valid NC social work license (LCSW) preferred and valid driver’s license.

Responsibilities

• Perform initial and ongoing psychosocial assessments of patients/family/caregiver.
• Develop and evaluate plan of care for each patient/family/caregiver unit.
