Job summary

Advocate Health is seeking a Hospice Social Worker to provide psychosocial evaluations, counseling, and support to patients and families in the Manitowoc and Green Bay territories. The role collaborates with an interdisciplinary team within a full-time schedule.

Requires a Master’s in Social Work, Wisconsin licensure, and one year of medical/hospice experience preferred. Travel across service areas is expected.

Qualifications

• Master's Degree in Social Work from an accredited school.
• One year experience in a medical clinical setting or hospice/palliative care preferred.

Responsibilities

• Provides initial psychosocial evaluations and ongoing counseling.
• Assesses psychosocial needs and develops a social work plan of care.
• Evaluates hospice level of care with the interdisciplinary team.
• Supports patients and families in Manitowoc and Green Bay territories.
