Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Social Worker to deliver high-quality psycho-social care within an interdisciplinary team. The role emphasizes education, counseling, planning, and coordination with patients and families to maximize coping and comfort.

Ideal candidates hold an MSW (CSWE-accredited) with healthcare experience or a social work degree with supervised MSW eligibility. Licensure as required by Kentucky is expected.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year healthcare experience.
• Alternatively, a Bachelor’s degree in social work with 1 year healthcare experience and MSW supervision available.

Responsibilities

• Explain hospice services and Medicare benefits; obtain consent when requested.
• Submit documentation to facilities per policy and timelines.
• Participate as IDT member in care planning and implementation.
• Provide psycho-social support and counseling to patients and families; refer to external resources as needed.
• Identify intervention needs of other IDT members and communicate patient needs.
• Coordinate psycho-social care with IDT to ensure continuity and quality of care.
• Maintain updated care plans reflecting problems, goals, and interventions.
