UnitedHealth Group’s Community Hospice division seeks a Social Worker to provide medical social services under physician direction and an interdisciplinary group, addressing social and emotional factors that influence a patient’s health status.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work

Responsibilities

• Assist patient, family, physician and care team to understand social and emotional factors affecting health.
• Contribute to a comprehensive Plan of Care for patients daily.
• Instruct care team on community resources available to assist patients as needed.
