Job summary

Otterbein Hospice is seeking a Social Worker to provide psychosocial services to terminally ill patients and their families. The role emphasizes counseling, resource referrals, and care planning within our hospice program.

The ideal candidate has a Master’s in Social Work, an active Ohio license, and at least 1 year of healthcare experience in settings dealing with terminal illness. This position supports a compassionate, team-based approach to patient and family care.

Qualifications

• Master's degree in social work from an accredited program.
• Active Ohio Social Work license is required.
• Minimum 1 year of experience in a health care field dealing with terminal illness and families.

Responsibilities

• Conduct initial psychosocial and bereavement risk assessment within five days of hospice election.
• Refer to community resources and arrange financial assistance when indicated.
• Assist with placement planning for basic care, respite, or inpatient needs.
• Provide psychosocial counseling to patients, families, and caregivers.
• Update clinical documentation accurately and timely; assist with discharge planning.
