Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care addressing family/caregiver needs.
• Interpret and respect patient/family faith and cultural traditions in care planning.
• Educate patients and families to overcome barriers and tailor learning to their needs.
• Document assessments, financial assessments, and MSW interventions in the EMR.
• Collaborate with the inter-disciplinary team to implement the social work plan.
• Support decisions and communication of patient life-care preferences.
