Acts Hospice in Montgomery County, PA, is seeking a Hospice Social Worker to join our per diem team. You will develop, provide, document, and evaluate social work services for hospice participants and families as directed by the care plan.

You will counsel on life closure, dying, and grieving in collaboration with the IDT. The role requires a graduate degree in social work, current licensure, and at least two years of hospice or equivalent clinical experience.

Acts Hospice is currently seeking qualified candidates for our Social Worker position.

In this role, you will be responsible for developing, providing, documenting, and evaluating individualized social work assessment, counseling, and casework services to hospice participants and families as directed by the plan of care. Counsels hospice participants and families on issues related to self-determined life closure, safe and comfortable dying, and effective grieving, in collaboration with the Interdisciplinary Team (IDT).

Requirements

• Graduate degree in social work or related field
• Current licensure in the state where services are rendered
• Minimum of two years' experience in hospice or a transferable, clinical venue of social work practice such as oncology, geriatrics, or community health

Pay Range

$33.99 - $41.57 / hour. Starting rate will vary based on skills and experience.
