Job summary

Acts Retirement-Life Communities seeks a Hospice Clinical Director to lead planning, delivery, and evaluation of compassionate hospice care. You will coordinate care plans, advocate for patient and family needs, and drive quality improvement within the team.

The role requires a BSN, 2–3 years nursing experience (hospice preferred), active RN license, CPR, and a valid driver's license. Comprehensive benefits and competitive pay are offered.

Qualifications

• 2–3 years of professional nursing experience.
• Hospice, home health, geriatrics, oncology, or similar clinical setting preferred.
• Active RN license in good standing.
• Current CPR certification (or obtain within three months).
• Must possess and maintain a valid state driver’s license.

Responsibilities

• Coordinate planning, implementation, and delivery of hospice care.
• Evaluate effectiveness of hospice services and ensure timely implementation and ongoing evaluation of patient care plans.
• Advocate for physical, emotional, psychosocial, and spiritual needs of patients and families while promoting dignity and quality of life.
• Serve as a knowledgeable resource, advocate, and educator to patients, families, caregivers, and healthcare team.
• Support best practices and continuous improvement in end-of-life care.

Acts Hospice is currently seeking qualified candidates for our Hospice Clinical Director position.

In this role, you will be responsible for coordinating the planning, implementation, and delivery of high-quality, compassionate, and cost-effective hospice care. Evaluates the effectiveness of hospice services and ensures the timely implementation, coordination, and ongoing evaluation of individualized patient care plans. Advocates for the physical, emotional, psychosocial, and spiritual needs of patients and their families while promoting dignity, comfort, and quality of life throughout the end-of-life journey. Serve as a knowledgeable resource, advocate, and educator to patients, families, caregivers, and members of the healthcare team, supporting best practices and continuous improvement in the care of individuals with life-limiting and terminal illnesses.

Requirements

The ideal candidate will meet the following requirements:

• Bachelor of Science in Nursing (BSN) degree, either completed or currently in pursuit
• Minimum of 2-3 years of professional nursing experience
• Experience in hospice, home health, geriatrics, oncology, or a comparable clinical setting is strongly preferred
• Active Registered Nurse (RN) license in good standing with the applicable state Board of Nursing
• Current CPR certification (or obtain within three months of employment)
• Must possess and maintain a valid state driver’s license

Pay Range

$81,689.00 - $99,985.00 / year. Starting rate will vary based on skills and experience.
