Job summary

Acloché LLC seeks a Hospice Sales / Liaison to develop and manage referral relationships in Rossville and surrounding areas. This field-based role focuses on expanding hospice/home health referrals and census growth while coordinating with clinical teams.

The ideal candidate has hospice or home health sales experience, a robust local network of physicians and facilities, and a proven track record of meeting referral targets in a fast-paced healthcare environment.

Qualifications

• Hospice and/or Home Health Sales experience required.
• Minimum 3 years of healthcare sales, marketing, business development or referral development experience preferred.
• Established local contacts/referral sources preferred.
• Local market knowledge and proven performance must be demonstrated.
• Ability to develop relationships with diverse referral sources.
• Familiarity with CRM/contact management software preferred.
• Strong problem-solving and independent field work capability.
• Ability to thrive in fast-paced, relationship-driven healthcare environment.
• Reliable transportation and valid driver's license required.

Responsibilities

• Develop, expand and maintain strong referral-source relationships across the assigned market.
• Represent hospice services professionally with referral partners, patients, families and community partners.
• Identify new referral opportunities and strengthen existing accounts through outside sales.
• Collaborate with clinical and operations teams to support referrals and transitions into care.
