Clear Concepts, Inc. is seeking a Hospice Sales Liaison to develop and maintain referral networks in Rossville and surrounding areas.

The role requires strong local market knowledge and a proven ability to grow census through physician, hospital, discharge planner and SNF partnerships. Candidates should have hospice or home health sales experience, an established referral network, reliable transportation, and the ability to work independently in a fast-paced healthcare environment.

Qualifikationen

• Hospice and/or Home Health Sales experience required.
• Minimum 3 years of healthcare sales, marketing, business development or referral-development experience strongly preferred.
• Established local contacts/referral sources strongly preferred.
• Local market knowledge, job stability and proven, verifiable performance are must-haves.
• Ability to establish and expand relationships with diverse referral sources.
• Familiarity with CRM/contact management software preferred.
• Strong problem-solving skills and ability to work independently in the field.
• Ability to thrive in a fast-paced, relationship-driven healthcare environment.
• Reliable transportation, valid driver's license and current auto insurance required.

Aufgaben

• Develop, expand and maintain strong referral-source relationships across the assigned market.
• Represent hospice services professionally with referral partners, patients, families and community partners.
• Identify new referral opportunities and strengthen existing accounts through consistent outside sales activity.
• Work closely with internal clinical and operations teams to support appropriate referrals and smooth transitions into care.
