Qualifications

• Two years of nursing experience, or an equivalent combination of education and experience.
• Current MD nursing license.

Responsibilities

• Provide quality nursing care to patients with life-limiting illness.
• Participate in developing and implementing individualized plan of care.
• Evaluate patient/family responses and modify care plan appropriately.
• Provide education to achieve skills necessary for quality care.
• Document on appropriate chart and forms accurately and timely.
• Participate in Quality Improvement Program.
• Provide direction and supervision to Patient Care Partners and volunteers.
• Attend to patient and family at the time of patient’s death.

Education
