BAYADA Home Health Care is seeking a full-time Registered Nurse (RN) – Hospice to support our Windsor, VT team. This role focuses on compassionate, home-based hospice care, working with a multidisciplinary team to maintain patient comfort and dignity in their homes.

You'll enjoy a Monday–Friday schedule with no weekends or on-call responsibilities, a $20,000 sign-on bonus, and competitive pay based on experience.

Qualifikationen

• Current Registered Nurse license in Vermont and New Hampshire.
• At least one year of recent, verifiable clinical experience in medical-surgical nursing or related area.
• Home health and/or hospice experience preferred but not required.
• Basic computer skills; EMR experience preferred.
• Ability to travel to patient homes within assigned service area.

Aufgaben

• Make home visits to hospice clients within your geographic territory.
• Provide skilled nursing care including medication administration, wound care, treatments, and comfort-focused care.
• Assess patients' conditions and communicate changes to the clinical team.
• Implement and document physician orders accurately and promptly.
• Perform patient assessments and help coordinate individualized plans of care.
• Collaborate with the interdisciplinary team to support patients and families throughout the hospice journey.
