BAYADA Home Health Care in Norwich, Vermont, is seeking a Registered Nurse (RN) to join their Hospice team. You will provide skilled nursing care in patients' homes, manage medications, wound care, and comfort-focused treatment within an assigned geographic area.

Qualificações

• Current Registered Nurse (RN) license in Vermont and New Hampshire.
• At least one year of recent clinical experience in medical-surgical nursing or related area.
• Home health and/or hospice experience preferred but not required.
• Basic computer skills; EMR experience preferred.
• Ability to travel to patient homes within the assigned service area.
• A compassionate approach to patient care and a commitment to clinical excellence.

Responsabilidades

• Making home visits to hospice clients within your assigned geographic territory.
• Providing skilled nursing care, including medication administration, wound care, treatments, and comfort‑focused care.
• Assessing patients' conditions and communicating changes to the appropriate clinical team members.
• Implementing and documenting physician orders accurately and promptly.
• Performing patient assessments and helping coordinate individualized plans of care.
• Collaborating with the interdisciplinary team to support patients and their families throughout the hospice journey.
