Qualifications

• 2+ years of nursing experience is preferred.
• Willingness to work a flexible schedule including evenings and weekends.
• Valid Vermont Driver’s License and car insurance required.

Responsibilities

• Assess patient/caregiver needs to provide quality care.
• Develop and communicate a realistic plan of care for each patient.
• Serve as Case Manager for assigned patients.
