Job summary

Enhabit Home Health & Hospice is seeking a qualified Nurse Manager to coordinate hospice services for our patients. You will ensure care is delivered to the highest standards and facilitate collaboration among physicians, caregivers, and field staff to maintain continuity of care.

This full-time, on-site leadership role requires RN licensure, CPR certification, and a valid driver license. We offer generous benefits, 401(k) matching, and continuing education opportunities.

Qualifications

• RN license and current state licensure.
• Minimum two years nursing experience; hospice/home health preferred.
• CPR certification required.
• Valid state driver license and car insurance.
• Strong communication and coordination skills.
• In-office leadership role with supervision of field staff.

Responsibilities

• Coordinate hospice services to maintain standards of practice and company procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visit.
• Facilitate collaboration between physicians, referral sources, patients, caregivers, and staff to maintain continuity of care.
• Schedule patients to field clinicians and ensure effective communication with all parties.
