Enhabit Home Health & Hospice is seeking aRegistered Nurse manager to oversee hospice services, ensuring care coordination and adherence to standards. You will review patient information for start of care and recertification while coordinating with physicians, referrals, patients and caregivers.

Two years of nursing experience, active CPR certification, and a valid state license are required; in-office attendance is essential to supervise staff and ensure high-quality patient care.

Qualificações

• RN licensed to practice in state; CPR certification required.
• Minimum two years nursing experience; hospice/home health preferred.
• Knowledge of federal/state/local laws governing home care operations.

Responsabilidades

• Coordinate hospice services to patients per standards and procedures.
• Review and approve start of care, recertification, resumption of care, or evaluation visit information.
• Facilitate communication among physicians, referral sources, patients, caregivers and staff to ensure care coordination and continuity.
