Job summary

Amedisys is seeking a PRN Registered Nurse in the Aiken, SC area to deliver home health and hospice services with compassionate, patient-centered care. You will assess patients, develop and modify care plans, and collaborate with the care team to ensure quality outcomes.

This role requires an active RN license in the appropriate state, CPR certification, and a valid driver's license. Prior nursing experience is preferred, with opportunities for growth in a flexible, patient-focused environment.

Qualifications

• Current RN license to practice in the state(s) assigned.
• 1 year nursing experience preferred; new graduate considered for RN program.
• Current CPR certification is required.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care, ensuring continuity of patient care with the care team.
• Communicates patient progress to clinical manager, physician, and care team.
• Maintains patient care progress records accurately and timely.
• Records pain and symptom management changes and outcomes.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed.
• Performs on-call duties and on-call services to patients/families as assigned.
• Performs other duties as assigned.
