Job summary

Amedisys is seeking a registered nurse for full-time day shifts in home health. You will assess new admissions, develop and monitor plans of care, and coordinate with physicians and the care team to ensure patients heal in the comfort of their homes.

Qualifications

• Current RN license, state-specific.
• Current CPR certification.
• Valid driver''s license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the care team.
• Regularly communicates patient progress to clinical manager and physicians.
• Develops and maintains patient care progress records with accuracy and timeliness.
• Records pain and symptom management changes and outcomes.
• Educates families on patient condition and needs.
• Performs on-call responsibilities as assigned.
• Performs other duties as required.
