Job summary

Residential Home Health and Residential Hospice is seeking a part-time Hospice RN to provide end-of-life care in patients' homes around Traverse City, MI. The role includes weekday, weekend, and after-hours coverage with potential to move to full-time.

You will collaborate with an interdisciplinary team, perform patient assessments, manage symptoms, educate families, and maintain accurate EMR documentation. Travel about 20% of the day to patient locations; active RN license required.

Qualifications

• Active RN license in the state you work.
• At least 2 years RN experience; hospice experience preferred.
• Valid driver's license; able to drive ~20% of the day.

Responsibilities

• Deliver patient-centered hospice care with an interdisciplinary team.
• Assess patients, manage symptoms, plan end-of-life care.
• Educate and support patients and families during the journey.
• Think critically, document thoroughly in EMR.
• Work independently, travel to patient homes within territory.

We are hiring a Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Traverse City, MI area. This position is will assist with covering weekday, weekend, and after hours rotation and is eligible for progression into a full-time role.

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations
