Job summary

Residential Home Health and Residential Hospice in Howell, MI is seeking a Home Health RN to provide skilled nursing care in patients' homes, collaborating with an interdisciplinary team to support autonomy and growth. A $10,000 first-year success bonus is available for achieving key milestones.

The RN must hold an active RN license in the state and be willing to travel to patient locations within the assigned territory; 40 hours per week with comprehensive benefits and growth opportunities.

Qualifications

• Active Registered Nurse, RN license in the state you work.
• One year of RN experience and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
